5S Audit
The 5S Audit module in ProBeya digitizes workplace organization assessments based on the 5S methodology: Sort, Set in Order, Shine, Standardize, and Sustain. Teams create reusable checklist templates, conduct scored audits on the shop floor, and automatically generate follow-up actions for low-scoring items.
Audit scores are tracked over time, enabling trend analysis and radar chart visualization of 5S maturity across categories.
5S Categories

Creating an Audit Checklist
A checklist template defines the assessment criteria used during audits. Each template can be reused for multiple audit sessions.
- Navigate to a board and open the 5S Audit tab.
- Click + New Checklist.
- Enter a name (e.g., "Production Line A — 5S Checklist").
- Add assessment items, each with:

- Click Save. The checklist is now available for audit sessions.
Organize items by category so the auditor can work through each 5S pillar systematically during the walk. Aim for 4-6 items per category for a balanced assessment.
Conducting an Audit
Starting a Session
- In the 5S Audit tab, click Start Audit.
- Select the checklist template to use.
- Optionally set the audit date (defaults to today).
- Click Begin. A new audit session opens in in_progress status.
Scoring Items
For each checklist item, the auditor assigns a score:

|---|---|
| 1 | Very poor — significant non-conformance |
| 2 | Poor — multiple issues observed |
| 3 | Acceptable — minor issues, room for improvement |
| 4 | Good — meets standard with minor observations |
| 5 | Excellent — fully compliant, best practice observed |
For each item, the auditor can also:
- Add a comment explaining the score rationale.
- Attach a photo as evidence (uploaded to S3 storage).
Scores can be corrected during the session by re-scoring the same item. The latest score always takes precedence.
Completing the Audit
- After scoring all items, click Complete Audit.
- The system validates that every checklist item has been scored.
- The final total score and percentage are calculated.
- The session transitions to completed status.
All checklist items must be scored before completing an audit. Partial audits cannot be finalized.
Generating Actions from Audit Results
Low-scoring items can be automatically converted into tracked action items:
- Open a completed audit session.
- Click Generate Actions.
- Set the score threshold (default: 3). Items scoring below this threshold will generate actions.
- Click Confirm.
For each low-scoring item, an action is created with:
- Title:
[5S Audit] Improve: {question text} - Description: Score details and auditor comment
- Priority: High for scores of 1, Medium for scores of 2
- Source:
audit(for traceability)
Score History and Radar Chart
Trend Analysis
The score history view shows how audit performance evolves over time:
- Line chart plotting total score percentage across completed sessions.
- Date filtering to focus on specific time periods.
- Checklist filtering to track a specific template's performance.
Radar Chart
The radar chart displays per-category scores, giving a visual overview of 5S maturity:
- Each 5S category forms one axis of the radar.
- The current audit score is plotted as a filled polygon.
- Previous audits can be overlaid for comparison.
This visualization instantly highlights which 5S pillars need the most attention.
